How does HeartCare protect the privacy of my health information,

Please go to the HeartCare Home Page to link to and read our Notice
of Privacy Practices (NPP). The Notice describes in some detail how
we protect patient privacy consistent with the law, including HIPAA.
Please also feel free to ask for a hard copy of our NPP when you come
for your appointment.
We maintain patient records in compliance with regulations of the
Virginia Board of Medicine which require that your health records be
maintained for a minimum of 6 years from the last date of patient encounter.
May I pre-register for my appointment on-line?
Although we do not make appointments and handle
registration by email, registration forms are provided for
your convenience under the Appointment Form tab on our home page.
We encourage you to download and complete the forms in advance
to bring with you at the time of your appointment to make your check-in
efficient and comfortable.
You will see that the registration form asks for demographic and insurance
information.
If you are an established patient, it is not necessary to complete
the registration form for each visit. But a section on the second page
asks you to complete an annual insurance verification.
The second form is the Acknowledgement of Receipt of the Notice
of Privacy Practices which we must ask patients receiving the NPP for
the first time to sign and submit. This form also follows the NPP posted
on our website.
